New Patients: Your first visit will consist of a thorough medical intake of present health concerns and past medical history, pertinent physical exam and review of prior labs / diagnostic tests. Further tests may be ordered at this time. Together, we will discuss the best approach to treatment, which may involve collaboration with your other health care providers or specialists if warranted. Initial appointments are typically 45-60 minutes, depending on the complexity of the case.
Forms: Our forms are found online in the patient portal. Once you have established your appointment, we will provide you with a link to the patient portal, where you may fill out forms and upload labs (should you have them). It is very helpful to have these completed prior to your first appointment.
“Meet & Greet” visit: We know finding a new doctor can be challenging. We offer a complimentary 10 minute phone visit to determine if our clinic is a good fit for your needs. Call 206.300.1337 to schedule.
Insurance: We do not directly bill insurance. At the end of your visit, we provide an invoice with diagnostic codes that may be submitted to your insurance for reimbursement. Many of our patients use HSA (Health Savings Accounts) for their visits. Labs and other diagnostic tests ordered by Dr. Ellis are billed directly through insurance.
- 15-minute check back / medication check – $75
- 30-minute check back – $150
- 45-minute new patient / return visit – $225
- 60-minute new patient visit – $300
Cancellation or Rescheduling: If you are unable to make it to your appointment, please call 206.300.1337 to reschedule. We require at least 24 hours notice so we may offer the appointment time to another patient. Rescheduling within 24 hours will also allow you to avoid any cancellation fees. Our late cancellation fee is $50.
